What if I need additional forms from my landlord or airline?
If you did not order any forms with your original package, please contact us for all future form requests. Additional forms can be completed for existing clients for $49 each. Please note that airline forms may require signatures from your veterinarian as well, so be sure that your animal meets all requirements for travel.
When will I receive my items?
You will receive an email with your completed documents upon approval. Once submitted, your evaluation will be reviewed within 24 hours. Please note that we are closed on Sundays and Federal holidays, and orders placed on holidays or weekends will be processed on the next business day. If you ordered hardcopies or other accessories, they will be shipped out upon approval with your selected delivery speed and options. If you requested a tracking number, this will be sent to you by email when your order ships.
Can I cancel my order?
As this is a professional mental health service, once you submit your evaluation for review, The $129 Evaluation fee is non-refundable. Review our return policy for more information.
What if my therapist denies my evaluation?
If your evaluation is denied by our therapist, we will contact you directly, and issue you a full refund.
